Client Consent Form and After Care Instructions (Downloadable File)

This full-color two-part downloadable brochure contains useful information about tooth jewelry.  The Client Consent is a form signed by the client prior to a tooth jewelry application and confirms that he or she agrees to the procedure and is aware of any risks that might be involved.  The downloadable brochure also includes Aftercare Instructions which you can tear off and give to your client at the end of their appointment.  There is space at the bottom for a company label or stamp.
